An unannounced complaint investigation was initiated along with Licensing Inspector Cathy Aylor on 11/13/23 in response to a complaint received in the licensing office regarding allegations of inadequate care and protection of children. Based on a review of available documentation and interviews, the Licensing Inspector was unable to substantiate the allegation due to insufficient evidence, therefore the compliant was determined to be invalid. Standard #: 8VAC20-780-330-B Complaint related: No Description: Based on observation, it was determined that there was an insufficient fall zone, clear of all hazardous obstacles, to the rear of the swings.
Evidence: The Licensing Inspectors observed approximately 18 oversized outdoor building bricks, a large construction bucket and three blocks between 2 to 5 feet to the rear of the swings. These obstacles were in fall zone of the swings.Plan of Correction: Removed outdoor oversized building blocks and buckets.
