Comments:
A monitoring inspection was conducted January 14, 2023. The licensing inspector was onsite, completing the inspection from approximately 11:00am until 1:30pm. There were 59 children present, ranging in ages from three months to five years, with 11 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, and medication. A total of six children's records and six staff members? records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and a violation was documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-160-A
Description: Based on review of six staff records, observation, and documentation review, the center did not ensure two staff members submitted documentation of a negative tuberculosis (TB) screening that had been completed within the last 30 calendar days prior of the date of employment.
Evidence:
1. The record for Staff #1 (date of employment August 22, 2023) contained documentation of a negative TB screening dated September 16, 2023.
2. The record for Staff #3 (date of employment November 22, 2023) contained documentation of a negative TB screening dated June 1, 2022.
3. A member of management confirmed this information during the inspection.

Plan of Correction: We did not realize the requirements for obtaining the TB screening had changed. We will ensure all staff going forward obtains the screening within the required timeframe.

Standard #: 8VAC20-780-240-A
Description: Based on review of six staff records, observation, and documentation review, the center did not ensure one staff completed the Virginia Department of Education-sponsored orientation course within 90 days of employment.
Evidence:
1. The records of Staff #1 (date of employment August 23, 2023) did not contain documentation of completing the Virginia Department of Education-sponsored orientation course within 90 days of employment.
2. Staff #1 was observed working and counting in staff-to-child ratio during the inspection.
3. A member of management confirmed this information during the inspection.

Plan of Correction: Staff #1 has started the course but needs to complete it.

Standard #: 8VAC20-780-270-A
Description: Based on observation, the center did not maintain all areas and equipment of the center, inside and outside, in a clean, safe, and operable condition.

Evidence:
Three tricycles located on the side outdoor playground, were missing the pedals, leaving the metal spokes exposed.

Plan of Correction: We have already ordered replacement pedals and will remove the tricycles until the pedals arrive.

Standard #: 8VAC20-780-550-F
Description: Based on documentation review and interview, the center did not ensure Lockdown procedures were practiced at least annually.
Evidence:
The drill log documenting practiced emergency drills did not indicate a lockdown drill was practiced.
A member of management confirmed a lockdown drill had not been practiced.

Plan of Correction: We will make sure to complete the required lockdown drills.
