A monitoring inspection was conducted on 12/21/2023. There were a total of 50 children with 12 staff members present. Children were observed during craft time, gym class, and playing with toys. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today in order for it to be included on the website. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Standard #: 22.1-289.035-B-4 Description: Based on a review of staff records, the CDC did not ensure they obtained a copy of the results of sex offender registry check, and a search of child abuse and neglect registry for any state in which the individual has resided in the preceding five years.
Evidence:
Staff #3 (date of hire 06/22/2022) did not have out of state sex offender or child abuse and neglect results.Plan of Correction: We have reached out to the state and submitted paperwork to receive the background checks.
Standard #: 8VAC20-780-160-C Description: Based on a review or staff records, the CDC did not ensure at least every two years from the date of the first initial screening or testing a follow up tuberculosis screening was submitted.
Evidence:
Staff #1 had a TB test or screening that expired on 08/22/2023.
Staff #5 had a TB test or screening that expired on 11/16/2023.Plan of Correction: The staff members got updated TB screenings.
Standard #: 8VAC20-780-60-A Description: Based on a review of child records, the CDC did not ensure each child enrolled had the required information.
Evidence:
Child #1 (start date July 2023) did not have documentation of proof of identity.Plan of Correction: We have reached out to the parents to bring in proof of identity for the child.
