Comments:
An unannounced monitoring inspection was conducted on 12/15/23 from 9:28 a.m. to 10:30 a.m. with the Director. There were 35 children in care, ranging in age from four to five-years-old, supervised by five staff. The children were observed making holiday crafts and preparing for a holiday party. Five child records and five staff records were reviewed. One medication and authorization form were reviewed. The attendance, menu, and emergency drill log were reviewed. The first aid kit, flashlight, and battery -operated radio were observed. Areas of non-compliance are identified in the Violation Notice. Description: Repeat Violation

Based on observation and interview the center did not obtain written authorization from the child?s physician and parent for long-term prescription medication.

Evidence: The emergency medication authorization form for Child #6 authorized the center to administer Auvi-Q, the medication on site for this child was epinephrine. A new medication authorization form, with the correct medication, is required and must be signed by the physician and parent when medications change.
