Comments:
An unannounced monitoring inspection was conducted on 12/11/23 from 1:00pm to 3:45pm. At the time of the inspection there were 32 children in care and 5 staff members. Children were observed napping and eating afternoon snack. Records were reviewed for five children and five staff members. Medication, evacuation drills, emergency supplies and other required records and postings were also reviewed. The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice and discussed during the exit interview.

Violations:
Standard #: 8VAC20-780-160-A
Description: Based on a review of 5 staff records, it was determined that the center did not ensure that each staff member shall submit documentation of a negative tuberculosis screening at the time of employment and prior to coming into contact with children.
1.The record for staff #5 (hired 12/4/2023) did not contain documentation of a negative TB screening. The staff member was working with children on the day of this inspection.
2.The Director confirmed the TB screening was not in the staff members record.

Standard #: 8VAC20-780-60-A
Description: Based on review of children?s records and interviews, it was determined that the center did not ensure that each child?s record contains all required information.
Evidence:
1.The record for child #2, #3, and #5 did not contain parent work phone numbers.
2.The record for child #2 (enrolled 10/23/23) and child #4 (enrolled 8/25/23) did not contain documentation of the health information (physical examination) required by 8VAC20-780-140.
3.Staff #1 (Director) confirmed the information was not in the children?s records.

Description: Based on observation and interviews, it was determined that the facility did not ensure that all areas and equipment of the center shall be maintained in a safe and operable condition.
1. The round carpet in the 2-3 year old classroom was frayed along the edges of the carpet.

Description: Based on observation and interviews, it was determined that the facility did not ensure that hazardous substances such as cleaning materials were kept in a locked place using a safe locking method that prevents access by children.
1. In the 2-3 year old classroom there was Lysol and sanitizer in a cabinet that had a child safety latch that was not engaging properly to prevent access.
2. The Director confirmed the cabinet?s safety latch was not engaged.

Description: Based on observation and interviews, it was determined that the center did not ensure that shelter in place procedures were practiced a minimum of twice per year.
1.The practice drill records did not contain documentation that the shelter in place drill was practiced twice in the past 12 months.
2.The director confirmed the shelter in place drills were not documented.
