Comments:
An unannounced monitoring inspection was conducted on 12/14/23 from 7:50 a.m. to 9:33 a.m. with the Director. There were 29 children in care, ranging in age from two to five-years-old, supervised by four staff. The children were observed in free play and eating breakfast. Six child records and five staff records were reviewed. The Center has six staff with current certification in CPR and First Aid, and three staff trained in Daily Health Observation Training. Two medications and authorization forms were reviewed and the center has four staff current in Medication Administration Training (MAT). Required postings were observed. The attendance, menu, and emergency drill log were reviewed. The first aid kit, flashlight, and battery- operated radio were observed. The most recent Fire Inspection on file was dated 11/04/22 and the most recent Health Inspection on file was dated 02/02/23. Breakfast served was an english muffin, cheese and oranges. Areas of non-compliance are identified in the Violation Notice. Violations:
Standard #: 8VAC20-780-270-A
Description: Based on observation, not all areas of the center were maintained in a clean, safe, and operable condition.

Evidence:
1. On the playground a broken bucket and shovel with sharp edges and a chalk board measuring approximately 8 feet by 3 feet was hanging on the fence, the wooden frame surrounding the chalk board was deteriorated, the bottom part of the wooden frame was broken with sharp points, creating an impalement and scratch hazard.

2. A broken blind on the door of the Hedgehogs classroom was observed, the blind had been split in half with sharp edges creating an impalement and scratch hazard.

Plan of Correction: 1. Bucket and shovel were removed immediately at time of visit from the playground. Maintenance was contacted about the chalkboard. It will be removed from the playground by 01/15/2024; in the meantime, the edges that were broken have been covered.

2. These blinds were less than two weeks old at the time of inspection and maintenance team was contacted and the blinds were replaced again on 12/18/2023.
