An unannounced non-mandated monitoring inspection was conducted on 12/20/2023 and concluded with an electronic record on 12/22/2023. There were 41 children, ages 2 years- 9 years, 6 staff members providing direct care and supervision and various support staff present during the inspection. The children were observed during rest time, arrival from school, self-directed activities, and departure. The inspector reviewed compliance in the areas of administration, staff qualifications and training, and background checks. A total of 3 children?s records and 3 staff records were reviewed. The inspector arrived for the inspection at approximately 2:45 pm and departed approximately 3:15 pm.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Standard #: 8VAC20-780-160-A Description: REPEAT VIOLATION
Based on record review, the center failed to ensure that each staff member shall submit documentation of a negative tuberculosis screening at the time of employment and prior to coming into contact with children and the documentation shall have been completed within the last 30 calendar days of the date of employment.
Evidence:
1. Staff 1 had a hire date of 12/08/2023. The date on the tuberculosis screening in the record was 10/09/2023. The tuberculosis screening had not been completed within the last 30 calendar days of the date of employment.
2. Staff 2 had a hire date of 12/15/2023. The date on the tuberculosis screening in the record was 10/30/2023. The tuberculosis screening had not been completed within the last 30 calendar days of the date of employment.Plan of Correction: Provider will ensure TB screening has been completed within the last 30 days before hire.
