An unannounced monitoring inspection was conducted on 12/14/23 from 11:15am to 1:50pm. At the time of the inspection there were 40 children in care and 7 staff members. Children were observed watching holiday movies and eating lunch. Records were reviewed for six children and five staff members. Medication, evacuation drills, emergency supplies and other required records and postings were also reviewed. The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice and discussed during the exit interview.

Based on record review, it was determined the facility did not ensure that a sworn statement or affirmation is obtained prior to the first day of employment or beginning to serve as a volunteer.

Standard #: 8VAC20-780-160-C Description: Based on a record review of five staff records it was determined that the center did not ensure that at least every two years from the date of the initial screening or testing, staff members shall obtain and submit the results of a follow-up tuberculosis screening as stated.

Standard #: 8VAC20-780-40-M Description: Based on observation and interviews, it was determined that the facility did not ensure that the current list of children?s allergies, sensitivities, and dietary restrictions are maintained in a way that is accessible to all staff who work with children and kept confidential in each room or area where children are present.

Standard #: 8VAC20-780-280-B Description: Based on observation and interviews, it was determined that the facility did not ensure that hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1. In the classroom #2 there was a can of TNT foaming disinfectant cleanser in an unlocked cabinet.
2. In Classroom #1 there was a can of Lysol disinfectant spray and sanitizer on top of a cabinet.
