Comments:
An unannounced monitoring inspection was completed on Thursday, December 28, 2023 at 10:15 am until 11:55 am at the center. There were 7 children present, ranging in ages from infancy to 11 years, with 5 staff supervising and 1 administrative staff at the facility. The inspectors completed observations of children in care in the classroom engaged in free play activities with age appropriate equipment, interacting with peers and staff, completing handwashing, and eating lunch. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special services, background checks, and nutrition. A total of 3 child records and staff records were reviewed. The focus of the inspection was to determine whether the provider had corrected or is in the process of correcting previously cited violations in the areas of standards or laws referenced above. Any non-compliant elements of standards or laws are documented in the Violation Notice of this inspection.
Pursuant to 8VAC20-820-410, General Procedures and Information for Licensure, the provider requested that the Department consider entering into a Consent Agreement in lieu of adverse action to deny the renewal application to operate a licensed child day center. On November 7, 2023, the provider submitted a final Compliance Plan to address the standards and law documented on a Notice of Intent to Deny Renewal application for a License to operate a August 28, 2023 issued. The provider demonstrated that they have followed their Compliance Plan actions. If there are any questions regarding the inspection please contact me inspector, Tiffany Jones at 757-403-3045.

Standard #: 8VAC20-780-130-A
Description: Based on review of children's records and interview, the center did not ensure to obtain documentation of a immunizations before the child can attend the center.

Evidence: In Child#3's record (), the immunization documentation was not completed with the dates. Interview with staff confirmed the immunization documentation received for review during the inspection.

Plan of Correction: The center's director will ensure that children will not attend without all the paperwork on file going forward. The child's file will be updated before he attends again. We will be checking paperwork for current students.
