Comments:
A monitoring inspection was conducted on 10/24/2023 from approximately 10:47 a.m. to 4:55 p.m. There were 64 children present and 10 staff supervising. The inspector reviewed compliance in the areas of administration, qualifications and training, physical plant, staffing and supervision, programming, medication, special care and emergencies, and nutrition. Six child (6) records and six (6) staff records were reviewed. The information gathered during the inspection determined violations with applicable standards or law. Please sign and complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return within 5 business days. Your plan of correction should contain: 1)
steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s). Description: Based on record review, the provider failed to ensure that each child shall have a physical examination by or under the direction of a physician before the child?s attendance or within 30 days after the first day of attendance.
Evidence:
The record for C2, enrolled 09/18/2023 did not contain a physical examination.

Description: Based on child record review, the provider failed to ensure that two of six immunizations records for children were signed by a physician, designee, or official of a local health department.

Description: Based on observation and interview, provider failed to maintain staff-to-child ratios as required by 8VAC2-780-350.B.2.
Evidence:
Licensing inspector observed a group of 11 children aged 17 months (C5) to two years old with two staff. The required staff-to-child ratio for this age group is 1 staff to 5 children. Staff acknowledged being out of ratio based on the child aged 17 months.

Description: Based on observation and record review, the provider failed to ensure that at least one staff in an area where children are present shall have a current certification in cardiopulmonary resuscitation (CPR) and first aid.
Evidence:
Licensing inspector observed S4 providing single staff supervision of children on the playground and S4?s CPR and first aid certification located in the staff record expired 10/22/2021.
