Discussed mulch on the playground, logging training for staff, injury reporting website, and out of state background website with after school director.
- Comments:

A monitoring inspection was initiated on 12/14/2023 at approximately 3:35pm and concluded on 12/14/2023 at approximately 5:05pm. There were a total of 43 children with 4 staff members present. Children were observed during snack time, engaged in group activities and outdoor play time. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. record information check or a search of the child abuse and neglect registry from any state

Evidence: There were no results of a criminal history record information check or a search of
the child abuse and neglect registry from each state in which Staff #2 resided in the preceding
five years.Plan of Correction: Center will obtain a criminal history record information check and a search of the child abuse and neglect registry from any state in which the staff resided in the preceding five years.
Standard #: 8VAC20-780-160-C Description: Based on review of staff information, the center did not ensure that staff obtain and submit tuberculosis screenings at least every two years from the first initial screening.
Evidence: The most recent tuberculosis screening for Staff #2 was more than two year ago.Plan of Correction: Center will ensure that Staff #2 obtains and submits a tuberculosis screening.
