Comments:
A subsidy health and safety (SHSI) inspection was completed on 12/01/2023 from 11:00 am - 4:00 pm. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.
Information gathered during the inspection determined violations with applicable standards or law. Violations are identified on the violation notice.

Violations:
Standard #: 8VAC20-790-530-C
Description: Based on observation and interviews, the licensee did not maintain a written hard-copy record of daily attendance that documents for EACH GROUP of children, the arrival and departure of each child in care, as it occurs.

The program had 5 written lists of children present, organized on a series of clipboards that remained at the front table throughout the entire inspection.
The clipboards were delineated by the site that each child normally attends during the school session.
The rosters were not organized by groups consistent with the grouping of children in care during the inspection.
The clipboards listed the names of children in attendance, but did not document the departure of children as they left.
Staff providing supervision for a group of children did not have a group roster listing the group of children that they were responsible for.
One staff was observed using an electronic device to check each child out of the program as they left.

Plan of Correction: Staff rosters will be created based on reservations for care prior to the morning that children arrive. Names can be added or crossed through as needed.
The rosters will be organized consistent with the grouping of children while in care and the staff responsible for these children will have possession of this roster to allow them to conduct name to face headcounts as children transition from one space to another.
Children will be marked as present when they arrive and marked as gone as they leave the program or transition to another group (where their name will be added to the group roster).
The rosters will remain current throughout the day, indicating which children are presently in care in a specific group.

Standard #: 8VAC20-790-600-D-1
Description: Based on a review of records, the program was unable to demonstrate that each staff person maintains current certification in cardiopulmonary resuscitation (CPR) appropriate to the ages of children in care.
Evidence:
Six staff records were reviewed.
The record for staff #2, whose date of re-hire was 5/23/2023, did not contain documentation of completing certification in cardiopulmonary resuscitation (CPR).
The staff person in charge was unable to locate this documentation.

Plan of Correction: Staff #2 will be required to complete certification in cardiopulmonary resuscitation (CPR) within 60 days and documentation will be placed in the staff record.

Standard #: 8VAC20-790-600-D-2
Description: Based on a review of records, the program was unable to demonstrate that each staff person maintains current First Aid Certification.
Evidence:
Six staff records were reviewed.
The record for staff #2, whose date of re-hire was 5/23/2023, did not contain documentation of completing certification in First Aid.
The staff person in charge was unable to locate this documentation.

Plan of Correction: Staff #2 will be required to complete certification in First Aid within 60 days and documentation will be placed in the staff record.

Standard #: 8VAC20-790-600-F
Description: Based on a review of records, the licensee was unable to demonstrate that each staff completes the department's required annual health and safety update course.
Evidence:
Six staff records were reviewed.
The record for staff #2 contained the most recent health and safety update course which was dated 6/13/2022 and was completed more than one year ago.

Plan of Correction: Staff #2 will complete the annual health and safety update training within 30 days and submit the certificate of completion to the program director. All staff will be required to take the training annually and turn in certificates to be placed in their records.
