An unannounced focused monitoring inspection was conducted 11/29/2023 from 4:05pm through 6:00pm. The inspection was conducted for the purpose of determining compliance in the areas that led to the center being placed on a 6 month Provisional License. There were a total of 43 children present with 3 staff. Attendance, supervision, and 5 children's records and 2 staff records were inspected. Children were observed completing homework, eating snack, and playing games during the inspection. Areas of non-compliance are identified in the Violation Notice.

Based on record review, the center did not ensure that references as to character, reputation, and competency were checked before employment for all staff.

Standard #: 8VAC20-780-350-B-5 Description: Based on observation, the center did not ensure the staff to child ratio of 1:18 was followed for school age children.
Evidence: On 11/29/2023, from approximately 4:20pm to 4:35pm, there were 41 children with 2 staff. The youngest child in the group was 5 years old. A third staff arrived at 4:35pm.Plan of Correction: Not available online. Contact Inspector for more information.
