Technical Assistance:
Discussed previous schools listed and discussed the staff's yearly training on the emergency preparedness plan.

Comments:
An unannounced renewal inspection was conducted on January 8, 2024 from 9:55am-11:55pm. At the time of entrance, 55 children were in care with 14staff members present. The inspector reviewed 2 children?s records and 2 staff records. Children were observed playing in centers, playing with play dough, making dot art, washing hands, playing pretend fishing at the water table and playing on the playground. Infants were being fed a bottle and being held, playing in a bouncy seat, sleeping and taking a walk in a buggy. The physical space, evacuation drills and attendance records were reviewed. The site was clean and organized. Interactions between the children and staff were positive. Areas of non-compliance are identified on the violation notice. Repeat violations were found and are identified within this report.

Standard #: 8VAC20-770-60-B
Description: Repeat Violation
Based on review of twor staff records, the center did not have a completed sworn
disclosure statement or affirmation prior to date of hire.
Evidence:
1. Staff #2 (Date of hire 9/05/2022) had a sworn disclosure statement dated 11/06/2022.

Plan of Correction: Teachers will complete sworn statement before hire date.

Standard #: 8VAC20-770-60-C-2
Description: Based on review of two staff records, the center did not obtain a central registry report within 30 days of employment.
Evidence:
1.Staff #2 (Date of hire 9/05/2022) did not have the cen tral registry results within 30 days of employment. It was received on 11/14/2022.

Plan of Correction: No teachers will be hire w/prior setting Background.

Standard #: 8VAC20-780-270-A
Description: Based on observation, equipment of the center inside were not maintained in an operable condition.
Evidence:
1. In the infant classroom, the foot operated trash can was not working properly. The staff member stated it was broke and they were getting a new one.

Plan of Correction: Replaced.

Standard #: 8VAC20-780-560-G
Description: Based on observation, the center does not follow all requirements when children bring food from home.
Evidence:
1. In the Preschool 3 classroom, 2 food items from home were not clearly dated and labeled in a way that identifies the owner and 1 food item from home was not dated.
2. In the Preschool 2 classroom, 1 food item from home were not clearly dated and labeled in a way that identifies the owner and 1 food item from home was not dated.

Plan of Correction: Teacher will be retrain in proper labeling of food in the classroom.
