Comments:
An unannounced renewal inspection was conducted with the director on January 3, 2024, beginning at 3:30pm and ending at 4:31pm. There were 12 school aged children in the care of 4 direct care staff. The children were observed playing games and interacting with one another. Five children?s records were reviewed. Three staff records were reviewed. The first aid kit, flashlight, and battery-operated radio were observed and in compliance. Two medications were reviewed and in compliance. The most recent evacuation drill was documented as 12/8/2023. The most recent shelter-in-place drill was documented as 11/1/2023. The most recent lockdown drill was documented as 9/5/2023. The most recent health inspection was dated 9/13/2023. The most recent fire inspection was dated 4/14/2023.

Standard #: 8VAC20-770-60-C-2
Description: Based on review of documentation and staff interview, it was determined that not all staff had
a documentation of a central registry finding search. Evidence: The records for staff #1 did
not have an a central registry finding letter.

Plan of Correction: The central registry search was requested but the documentation was not in the record. The documentation will be obtained from home office and placed in the staff's record.

Standard #: 8VAC20-780-130-A
Description: Repeat Violation: Based on a review of five children's records, the facility did not obtain documentation that each child has received the immunizations required by the State Board of Health before the child's first day of attendance. Evidence: The record for child A was missing documentation of a immunization by the first day of attendance which was 12/4/2023.

Plan of Correction: The documentation was requested from the parent prior to enrollment but it was never provided. The child was disenrolled from the program.
