An unannounced non-mandated inspection was conducted today, 1/08/2024, from 11:30am to 1:30pm. The inspection was in response to a complaint received on 01/02/2024 regarding Administration, Staff Qualifications, the Physical Plant and Special Services. The Licensing Inspector was accompanied by a Licensing Administrator for the duration of the inspection. The physical space, all snacks and 8 staff records were reviewed or inspected. The complaint was deemed Valid regarding Administration and Staff Qualifications. The complaint was deemed Not Valid regarding the Physical Plant and Special Services.

Standard #: 8VAC20-780-70 Complaint related: Yes Description: Based on record review, documentation that two or more references as to character and reputation, as well as competency, were not checked before employment. Evidence: 2 references were missing for 1 out of 8 records reviewed. Staff A was missing 2 references in their staff file. Plan of Correction: We will obtain references within 48 hours. Documentation for all future hires will be verified prior to hire.

Evidence: Documentation, demonstrating experience required by the job position, was not available for 1 out of 8 staff records. Staff B was missing documentation demonstrating programmatic experience. Plan of Correction: We will obtain required documentation within 48 hours. Documentation for all future hires will be verified prior to hire.
