Comments:
An unannounced monitoring inspection was conducted on January 8, 2024 between the hours of 9:15 am-12:00 pm. There were a total of 97 children with 18 supervising staff members present. Children were observed during toy play, dancing, circle time, handwashing, engaged in group activities and indoor gross motor play time. A selection of staff and children records, medications and other documents were reviewed. The physical space, emergency supplies, evacuation drills, attendance records, and the Emergency Preparedness Plan were reviewed. Areas of non-compliance are identified in the violation notice. Violations:
Standard #: 8VAC20-780-80-B-2
Description: Based on documentation review and interview, the center did not inform the department's representative as soon as practicable, but not to exceed two business days, of an injury to a child that occurred while in the child was under supervision of the center and required medical attention.
Evidence:
1.According to an Incident/Accident Report dated 11/20/2023, Child #11 was hit on the back and middle of their head with a toy by another student causing it to bleed. Child #11 was taken to the doctor's by the parent as a precaution and returned to the center in an hour.
2. Staff #6 confirmed that they did not contact the department's representative regarding the incident.

Plan of Correction: Moving forward, the center will contact the department's representative and complete a serious injury report online to inform the department of a serious injury in the center.

Standard #: 8VAC20-780-270-A
Description: Based on observation, areas and equipment of the center, inside and outside, were not maintained in a clean, safe and operable condition.
Evidence:
1. In the Toddler's classroom, the mat placed under the indoor slide was buckling and raised on the right corner of the mat. The buckling is a tripping hazard. There was peeling paint and sharp protruding edges where screws were removed on the wall. near the wooden wardrobe . The sharp protruding edges could entangle clothing or snag skin. There was also peeling paint on the wall near the carpet area.

Plan of Correction: The mat was removed from the Toddler classroom and a mat that did not bulge or buckle was placed under the slide at the time of the inspection.
A work order was placed to touch up the paint and caulk the wall to smooth the sharp edges.

Standard #: 8VAC20-780-280-D
Description: Based on observation and interview, cleaning and sanitizing materials were located above food and not stored in areas physically separate from food.
Evidence:
1. In the Toddler's classroom, a pack of crackers was located in a cabinet with two bottles of disinfectant and dish soap.
2. Staff #6 stated that the food belonged to staff and not the children.

Plan of Correction: The food was taken out of the cabinet at the time of the inspection.

Standard #: 8VAC20-780-520-C
Description: Based on documentation review, written parent authorization for diaper cream was not obtained.
Evidence:
1. In the two's classroom, Child #3's and Child #6's diaper creams did not have written parent authorization.
2. In the Toddler's classroom, Child #7's, Child #8's and Child #9's diaper creams did not have written parent authorization.

Plan of Correction: Diaper cream authorizations were completed and given to the parents to complete.
