A complaint inspection was initiated on 12/8/2023 and concluded on 12/29/2023. There were 33 children present with nine staff supervising, two administrative staff and one cook present on 12/14/2023. The inspector reviewed compliance in the areas of administration, staffing and supervision.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Please complete the ?plan of correction? and date to be corrected? for each violation cited on the violation notice and return it to me within five (5) business days from today. You will need to specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring gain; and 3) person or person(s) responsible for implementation.

Standard #: 8VAC20-780-350-B-1 Complaint related: Yes Description: Based on interviews and record reviews the center failed to ensure that the staff-to-children ratios are met whenever children are in care.
Evidence:
On 12/8/23 staff #1 was in the four-year-old room with 17 children ranging in age from one year to four years. Staff #1 was alone with these 17 children approximately 20-30 minutes. The ratio requirement for this age range would is one staff to four children.Plan of Correction: The director, assistant director, education coordinator, and third key will ensure the following.
Altering staff schedules to ensure adequate supervision at all times of the day, especially in the mornings. Additional staff training on ratio requirements and expectations for all age groups.
Preventing families from dropping off children when the classroom is at ratio. Altering staff schedules.
Standard #: 8VAC20-780-350-B-4 Complaint related: Yes Description: Based on interviews and record reviews the center failed to ensure that the staff-to-children ratios are met whenever children are in care.
Evidence:
Staff # 2 stated that during the months of October ? December 2023, on at least 6 different days, staff #2 had upwards of 20 children in the four-year old room between the hours of 6:00 a.m. ? 8:00 a.m. The ages of these children ranged from 3 to 10 years old. The ratio requirement for this age range is one staff to 10 children.Plan of Correction: The director, assistant director, education coordinator, and third key will ensure the following.
