Technical assistance was provided regarding Standard 550-P. Children's injury reports shall contain the following: the date and time of injury, name of injured child, type and circumstance of the injury, staff present and treatment, date and time when parents were notified, any future action to prevent recurrence of the injury, staff and parent signatures or two staff signatures, and documentation on how parent was notified.

An unannounced monitoring inspection was conducted 1/8/2024 from 8:30am through 10:50am. There were 34 children directly supervised by 7 staff and all classrooms were in ratio. The physical plant, outdoor playground area, programming, menu, 5 staff records, 5 children?s records, 3 medications, emergency drills, injury reports, emergency supplies, and policies were inspected. Children were observed handwashing, eating breakfast, and coloring. There was an adequate supply of age appropriate materials for the children. There was an adequate number of CPR/First Aid, Daily Health Observation, and MAT trained staff on site at the time of inspection. Areas of non-compliance are identified in the Violation Notice.

Standard #: 22.1-289.035-B-4 Description: Based on record review and staff interview, the center did not request a search of the child abuse and neglect central registry from each state where staff have lived within the last 5 years within 30 days of employment.
Evidence: Staff #5 (Date of Hire: 12/5/2023) has resided in North Carolina within the last 5 years and the center has not requested search results from the North Carolina child abuse and neglect central registry for Staff #5.Plan of Correction: Not available online. Standard #: 8VAC20-780-80-A Description: REPEAT VIOLATION
Based on observation and staff interview, the center did not maintain a written record of daily attendance for each group of children that documents the arrival and departure of each child in care as it occurs.
