A non-mandated remote inspection was conducted on January 12, 2024 to review documentation regarding the Consent Agreement requirements. The inspector reviewed receipt of acknowledgment forms of required parent notifications.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-820-410-C Description: Based on review of documentation, the center failed to implement the terms of the consent agreement.
Evidence:
1. The consent agreement signed on 09/12/2023 state that, "prior to Mighty Minds Little Hands International Preschool LLC, accepting any money for deposit and prior to enrollment of the any child" to provide evidence of acknowledgment of receipt of all required parent notifications.
2. Child #1's date of enrollment was 10/02/2023. The acknowledgment receipt was signed by the parent on 10/10/2023.
3. Child #2's date of enrollment was 10/02/2023. The acknowledgment receipt was signed by the parent on 10/12/2023.
4. Child#3's date of enrollment was 10/26/2023. The acknowledgment receipt was signed by the parent on 10/26/2023.Plan of Correction: The paperwork was given to the parent with all registration paperwork. The parents did not return the acknowledgment until after the child began enrollment. In November we implemented that any new enrollment could not begin until the acknowledgment receipt was in the child's record.
