Comments:
An announced initial inspection was conducted on January 11, 2024 from 10:00am through 1:00 pm. The physical plant, staff qualifications, background checks, policies and procedures and the emergency preparedness plan were reviewed. Classrooms were measured for space calculations to determine capacity. The classrooms are filled with supplies, equipment and age appropriate activities for children. The center is prepared to begin enrolling children. Areas of non-compliance are identified in the violation notice. Please contact me if you have any questions at karen.dickens@doe.virginia.gov.

Violations:
Standard #: 22.1-289.035-B-4
Description: Based on record review and staff interview, the center did not obtain a copy of the results of a search of the child abuse and neglect registry from a state in which an individual resided in the preceding five years by the end of the 30th day of employment.
Evidence:
1. Staff #3's record (Date of hire-09/25/2023) contained a sworn disclosure indicating that they lived in the state of Washington in the preceding five years. Staff #3's record did not contain results of a child abuse and neglect registry check from the state of Washington.
2. Staff #1 confirmed that a search of the child abuse and neglect registry from the state of Washington was not conducted by the end of the 30th day of employment.

Plan of Correction: The child abuse and neglect search for the state of Washington was conducted for staff the day of the inspection. The results of the search will be sent to the Licensing Inspector when they return.

Standard #: 8VAC20-780-160-A
Description: Based on record review, each staff member did not submit documentation of a negative tuberculosis(TB) screening completed within the last 30 calendar days of the date of employment.
Evidence:
1. Staff #4 (date of hire-01/08/2024) did not submit documentation of a negative TB screening completed within the last 30 calendar days of the date of employment. Staff #4's record contained documentation of a TB screening dated 03/04/2023.

Plan of Correction: Moving forward, the management team will ensure that documentation of tuberculosis test/screenings will be submitted at the time of employment and within 30 calendar days of employment.

Standard #: 8VAC20-780-70
Description: Based on record review, required records were not kept for each staff person.
Evidence:
1. Staff #2's record (Date of hire-08/21/2023) did not contain documentation of two or more references checked before employment.
2.Staff #3's record (Date of hire-09/25/2023) contained two or more references that were not checked before employment. Documentation of two or more references were dated 10/09/2023.
3. Staff #4's record (Date of hire-01/08/2024) did not contain documentation of two or more references checked before employment.
4. Staff #6's record (Date of hire-10/02/2023) contained documentation of two or more references that were not checked before the employment. Documentation of two or more references were dated 10/10/2023.

Plan of Correction: Moving forward, all references will be checked before employment. Documentation for the missing references will be placed in the staff records.
