An unannounced inspection was conducted 11/21/2023 and 12/01/2023 in response to a self reported incident that was received by the Virginia Department of Education, Office of Child Care Health and Safety. Interviews were conducted with staff relating to allegations of lack of supervision. Based upon a preponderance of evidence, the information gathered during the investigation supports the allegations of lack of supervision. Please complete the columns for 'description of action to be taken' and 'date to be corrected' for each violation cited on the violation notice, and then return a signed and dated copy to the licensing office within 10 business days. Standard #: 8VAC20-780-340-A Description: Based on staff interviews, the facility failed to ensure the care, protection, and guidance of children while under their supervision.
Evidence: Staff 1 and Staff 2 did not properly conduct name to face checks before leaving the classroom to take children outside, and left Child 1 in the classroom, unsupervised for approximately 6-15 minutes.Plan of Correction: As a result of a self-reported violation, both staff #1 and #2 received accountability for their actions. All staff received a retraining of proper Face to Name procedures at our monthly staff meeting on November 8th, 2023. Additionally, during this meeting, we highlighted best practices regarding observing each child being called.
A member of the management team will observe 1 Face to Name transition daily for the next 30 days
and will provide any necessary coaching on the spot to ensure clear expectations and understanding of the process.
Standard #: 8VAC20-780-340-F Description: Based on staff interviews, the facility did not ensure that children under 10 years of age were always within actual sign and sound supervision of staff.
Evidence: verified through staff interviews that Staff 1 and Staff 2 had left Child 1 unsupervised in the classroom for approximately 6-15 minutes. Child 1 was found by a third staff member who was conducting a tour of the facility, and was brought to the teachers.Plan of Correction: As a result of a self-reported violation, both staff #1 and #2 received accountability for their actions.
