A monitoring inspection was conducted on January 10, 2024 with the center director and staff. There were 7 children present, ranging in ages from 2 years to 4 years, with 2 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 3 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 22.1-289.036-A Description: Based on review of two agent records and staff interview, the facility failed to have Agents complete background checks in accordance with the Code of Virginia every five years. Evidence:
1. The record for Agent 1 contained the results of a search of the central registry dated 12/21/2017and a sworn disclosure statement dated 11/08/2017. 2. The record for Agent 2 contained the results of a search of the central registry dated 12/21/2017 and a sworn disclosure statement dated 11/14/2017. 3. The center director stated the background checks had not been repeated.Plan of Correction: The background checks will be repeated as soon as possible.
Standard #: 8VAC20-780-150-B Description: Based on review of 5 children's records, the facility failed to ensure that each child's immunization record received at enrollment was signed by a physician, his designee, or an official of a local health department. Evidence: The immunization record on file for Child 5 (date of enrollment 1/08/2024) was not signed by a physician or his designee. Plan of Correction: The provider will request a signed copy of the immunization records.
Standard #: 8VAC20-780-270-A Description: Based on observation and staff interviews, the licensee did not ensure areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.
Evidence:
1) All of the window frames, inside and outside, have areas of chipped and peeling paint.
2) At the front entry, there is a spot in the ceiling from water damage that has peeling paint.Plan of Correction: According to the owner, the center will be painted this winter.
Standard #: 8VAC20-780-330-B Description: Based on observation, the center did not ensure that where play equipment is provided, resilient surfacing complied with minimum safety standards described in the American Society for Testing and Materials. Evidence: there was no resilient surfacing under the two slides or in the fall zones of the 5 foot tall play equipment on the playground. Plan of Correction: According to the owner, the playground equipment will not be used until the proper resilient surfacing is placed.
