An unannounced monitoring inspection was conducted from 9:40 am to 11:45 am. There were 108 children in direct care and supervision of 18 staff members. The children were observed sleeping in cribs, engaged in gross motor skill activities and completing learning worksheets. 4 staff records, 4 children's records, medications, and other center documents were reviewed to assess compliance. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to the department within 5 business days from receipt. Please specify how the deficient practice will be or has been corrected. Please do not use staff names; list staff by positions only. Any plans of correction not received within 5 business days will result in the plans not appearing on the Violation Notice and public website. Standard #: 8VAC20-780-80-A Description: Based on records review, the center did not maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.
Evidence:
1. Staff # 1 stated that Child # 1 arrived at approximately 9:15 am. Child # 1's attendance was not recorded on the written attendance sheet and the digital application that center uses to record attendance.Plan of Correction: Not available online. Contact Inspector for more information.
Standard #: 8VAC20-780-350-B-1 Description: Based on observation, the center did not maintain the infant ratio of 1 staff to 4 children in the infant classroom.
Evidence:
1. In the Infant B classroom, at 9:51 am there were 5 infants with Staff # 2.
2. Staff # 3 arrived to the Infant B classroom at approximately 9:52 am.Plan of Correction: Not available online. Contact Inspector for more information.
Standard #: 8VAC20-780-350-B-3 Description: Based on observation, the center did not maintain the 1 staff to 8 children ratio in the 2's classroom.
Evidence:
1. Staff # 4 was observed to have walked into the toddler classroom from the 2's classroom to bring a child's belonging.
2. There were a total of twenty- 2 year old children in the classroom with Staff # 4 and Staff # 5 when Staff # 4 walked into the toddler room.
