Comments:
A monitoring inspection was conducted December 19, 2023. The licensing inspector was onsite, completing the inspection from approximately 10:30am until 1:00pm. There were 16 children present, ranging in ages from four months to 4 years, with three staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, and medication. A total of five children's records and five staff members? records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and a violation was documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-160-A
Description: Based on review of six staff records, observation, and documentation review, the center did not ensure one staff member submitted documentation of a negative tuberculosis (TB) screening that had been completed within the last 30 calendar days prior of the date of employment.
Evidence:
1. The record for Staff #5 (date of employment 11/29/2023) did not contain documentation of a negative TB screening.
2. Staff #5 was counted in the staff-to-child ratio during the inspection.
3. A member of management confirmed this information during the inspection.

Plan of Correction: Going forward, we will ensure new staff start with TB screenings obtained within the required timeframe.

Standard #: 8VAC20-780-60-A-8
Description: Based on review of five children?s records and interview, the center did not have a written allergy action care plan for one child diagnosed with a food allergy, to include instructions from a physician regarding the steps to take in the event of an allergic reaction.

Evidence:
The record for Child #1 (start date 09/25/2023) indicated the child has a food allergy. A food allergy action plan was in the record of Child #1. The food allergy action plan was not signed by a physician.

Plan of Correction: We returned the allergy action plan to have family obtain physician's signature.

Standard #: 8VAC20-780-240-A
Description: Based on review of six staff records, observation, and documentation review, the center did not ensure one staff completed the Virginia Department of Education-sponsored orientation course within 90 days of employment.
Evidence:
1. The records of Staff #3 (date of employment 06/20/2022) did not contain documentation of completing the Virginia Department of Education-sponsored orientation course within 90 days of employment.
2. Staff #3 was observed working and counting in staff-to-child ratio during the inspection.
3. A member of management confirmed this information during the inspection.

Plan of Correction: The staff member has started the training but has not finished it. We will ensure the staff completes the training.
