Technical Assistance:
As discussed, best practice suggests to maintain a hard copy of the emergency preparedness plan in order to review, update and make any changes to the plan and to also ensure an annual review is conducted with all staff.

Discussed the need to address the temporary solution to the interior paint in your classrooms. It is imperative that the maintenance of your building is maintained to prevent any hazards to the children in care. Areas of peeling paint were not currently accessible to the children, however a permanent solution to this maintenance issue is needed.

Comments:
An unannounced Renewal inspection was conducted today. A total of 43 children with 11 staff were onsite today in addition to management staff. A total of 4 classrooms were occupied with children. All four classrooms were observed. In the infant classroom, diapering procedures were observed, one infant was in their crib and the other infants were free playing on the carpet near staff. New enrolled children were adjusting to the toddlers class. A bucket of snow was brought in for a tactile activity for those interested while one staff was seated on the carpet reading book to some children. The intermediate and prek 2 class were busy with table top activities which included drawing with colored pencils, arranging alphabet pasta and completing a Martin Luther King activity about what they dream for. No medication needs at this time. Some topical ointments were onsite and authorizations were reviewed and found complete. A total of 4 children's files, 4 staff files, injury report log and staff certifications were reviewed. The recordation system was found organized and well maintained overall. Description: Based on child record review, documentation that a child has received immunizations required by the State Board of Health was not obtained before the child attended the center.
Evidence:
Child #3 began care on 10/24/2022. Immunizations maintained for this child was dated 12/01/2022.

Plan of Correction: No child will be enrolled without documentation of immunizations.

Standard #: 8VAC20-780-130-E
Description: Based on child record review, additional immunizations were not obtained every six months for one child under age 2 years.
Evidence:
Child #3's last documented immunizations were dated 12/01/2022 and 12/20/2023.

Plan of Correction: Child files will be reviewed to ensure that all children under the age of 2 years have updated additional immunizations on file.

Standard #: 8VAC20-780-140-A
Description: Based on child record review, a physical examination was not obtained within 30 days of a child's start date.
Evidence:
Child #3 began care on 10/24/2022. The physical exam on file was dated 12/01/2022.

Plan of Correction: Health exams will be obtained within 30 days of a child's start date. Reminders for parents will be made.

Standard #: 8VAC20-780-550-P
Description: Based on a review of the center's injury reports, reports were incomplete.
Evidence:
Injury reports did not include the time, date and method parents were notified on the majority of reports completed in 2023.

Plan of Correction: Staff will be reminded of the importance of documenting the entire injury report to include the time, date and method a parent was notified.
