Comments:
An unannounced monitoring visit and facility tour was conducted on 12/18/2023. At the time of inspectors' arrival there were 19 infant to preschool aged children in care with 3 staff members and 1 administrator. Children were observed interacting with staff, nap, snack and hand washing. A sample of 5 children's records and 3 staff records were reviewed. Injury reports were also reviewed. Areas of noncompliance are identified on the violation notice and were discussed with the director in an exit meeting at the conclusion of this inspection.

Violations:
Standard #: 8VAC20-780-570-A
Description: Based on observation and interview it was determined the center staff did not ensure when high chairs are utilized the protective belt was securely fastened.
Evidence:
1. There was an infant in a high chair without the protective belt secured.
2. Staff #2 confirmed the belt was not secured.

Plan of Correction: Director will retrain staff on using protective belts in high chairs and other equipment.
