Comments:
An unannounced renewal inspection was initiated and concluded on January 18, 2024. There were 46 children in care ages five months through five-years-old with nine teaching staff, one therapist and two administrators. During the inspection children and staff records were reviewed. Additionally, medication, emergency drills and equipment as well annual inspections and first aid kit and transportation were reviewed. Children were observed engaged in teacher and self-directed activities including play activities, table top manipulatives and outside play. Violations regarding hand washing, infant emergency supply and diaper disposal were observed, discussed with the program director during the exit meeting and appear on the violation notice.

Description: Based on observation the center failed to ensure that staff wash hands before diaper changes.
Evidence:
In the toddler room, Staff #2 did not wash her hand before changing the diaper of a child in care.

Standard #: 8VAC20-780-500-B
Description: Based on observation the center failed to ensure that staff dispose of diapers in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal.
Evidence:
In the toddler room, Staff #2 used her hand to lift the lid of the foot operated trash can to dispose of a soiled dipper and other items used during diaper changing.

Standard #: 8VAC20-780-570-I
Description: Based on observation and interview the center failed to ensure that a one-day's emergency supply of commercial formulas appropriate for children in care is maintained at the center.
Evidence:
1 - There were two containers of Enfomil with expiration date stamps 1DEC2023. The date stamps were on the bottom of the boxes
a. Written on the top of each box in black marker was the date 12/1/2023
2 - A single canister of Similac had a date stamp on the bottom of 1/1/2024.
3 - Staff #1 confirmed that the formulas had expired based on the date stamps.
