Technical Assistance:
Consultation was given on ensuring assessments are made to move younger children into the primary classes but maintaining the 1:10 ratio, references needed for new board officers and background checks are good for 5 years (Sworn Disclosure Statement, Central Registry as well as Fingerprints-Criminal Record Check).

Comments:
An unannounced renewal study was conducted. Observed three classrooms with one toddler group and two primary classes. Children were in ratio with direct care supervision. Observed children in small groups exploring and working on activities supported by Montessori equipment and materials. Reviewed a sample of children's records, medication on-site, one new staff record, emergency preparation, injury forms and plan, physical space and staff trainings. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 22.1-289.035-B-4
Description: Based on record review and administrative staff statement, a board officer does not have an out of state background check within 30 days of assuming a position on the board.
Evidence:
Board Officer #1 was missing a Maryland Central Registry Check.

Plan of Correction: State of MD will be contacted to trace background check and it isn't traced, another will be submitted.

Standard #: 8VAC20-780-60-A
Description: Based on record review, a child is missing 2 emergency contacts that are not the parent or guardian.
Evidence:
Child #1's two emergency contacts are the parents.

Plan of Correction: PArent has been contacted and the emergency contacts have been updated.

Standard #: 8VAC20-780-60-A-8
Description: Based on record review and administrative statement, a child has a documented possible allergy/intolerance but does not have a care plan.
Evidence:
Child #1 does not have a written plan for staff to follow in case of an exposure to the documented food.

Plan of Correction: Parent will be contacted regarding an update of the child's possible allergy and a care plan will be requested if it is still an active possibility.

Standard #: 8VAC20-780-270-A
Description: Based on observation, an area of the center has chipped or peeling paint.
Evidence:
The stairs leading into the program have peeling paint and the wall board near the Sunflower classroom has chipped paint.

Plan of Correction: The Church has been notified and repainting of the area has been requested.

Standard #: 8VAC20-780-350-Q
Description: Based on staff statement and record review, written assessments were not completed for two children to move up into the older classrooms.
Evidence:
Child #2 (two years) and Child #3 (two years) were missing written assessments from the classroom teacher and program director to verify they were developmentally ready to move up into the primary classrooms (ages 3-6)

Plan of Correction: Checklist for each child will be located and sent to inspector.
