Comments:
An unannounced monitoring inspection was conducted on January 17, 2024. There was a total of 33 children with 6 staff supervising. Children were observed playing with age-appropriate toys, resting and/or sleeping, diapering, handwashing procedures and preparing for afternoon snack. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, children and staff records, special care and emergencies and nutrition. The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 8VAC20-780-500-A
Description: Based on observation, the staff did not wash their hands with soap and running water before a diaper change.
Evidence:
1. Staff #2 did not wash their hands before a diaper change.
2. Staff #4 did not wash their hands before a diaper change.

Plan of Correction: I will retrain my staff on proper diapering procedures and will observe them to ensure they are following procedures.

Standard #: 8VAC20-780-500-B
Description: Based on observation, the staff failed to use the foot-operated leakproof storage system in such a way that neither the staff member's hand or soiled diaper touched the exterior surface of the storage system during diaper disposal.
Evidence:
1. Staff #2 used her hands to lift the foot-operated storage system when disposing of soiled diapers and paper towels.

Plan of Correction: I will remind staff they must use the foot-pedal storage system when disposing of soiled diapers. Violation was corrected during inspection.

Standard #: 8VAC20-780-570-E
Description: Based on observation and interview, the prepared infant formula was not dated and labeled with the child's name.
Evidence:
1. There were 3 bottles of formula not dated and labeled with a child's name.
2. Staff #4 confirmed the bottles of formula were not labeled for Child #4.
3. The director confirmed the bottles of formula were not labeled.

Plan of Correction: I will remind staff to ensure parents have labeled all bottles and if not, to add the label immediately. Violation was corrected: the staff labeled the bottles of formula during inspection.
