1) 160B: acceptable forms of documentation of TB screenings are:
a. A clearance statement signed by a physician, the physician's designee or an official of the local health department
b. The results of a negative TB skin test. The documentation shall include the date the test was given and the results of the test and by signed by a physician, the physician's designee or an official of the local health department
c. The results of a chest x-ray negative for active tuberculosis disease. The documentation shall include the date of the test and location where the examination was performed.
Discussed:
Crib mattresses shall be maintained at the lowest level for children that can sit up or stand. Best practices are to always have the crib mattress at the lowest level for safety.
Department of Social Services forms are out dated, and no longer in use as of 7/21. If department forms are used, they shall say Department of Education on them. See website for current model forms.

Comments:
An unannounced renewal inspection was conducted from 12:15 - 2:45pm with the center director. There were 39 children, ranging from six months to five years of age, present with nine staff supervising. Children were observed eating lunch, preparing for nap, and napping. Infant care was also observed. Lunch served today: cheese pizza, green beans, pineapple and milk. Five child records and six staff records were reviewed, along with the emergency drill log, written daily attendance, written allergy list, required posted information, annual fire and health inspections and three emergency medications. Standard #: 22.1-289.035-B-4
Description: Based on review of six staff records, it was determined that no out of state child abuse and neglect search was requested within 30 days of hire, and no out of state criminal history name check was conducted prior to hire, for all staff who stated they have lived outside of Virginia in the past five years. Evidence: the record for staff #1, (date of hire: 10/18/23) who indicated that they have lived out of the state of Virginia in the past five years, had no documentation of an out of state central registry search or a criminal history name check.

Plan of Correction: The out of state background check was sent on 1/23/2024.

Standard #: 8VAC20-780-140-A
Description: (Repeat violation) Based on a review of five child records, the facility did not obtain documentation that each child has received a physical examination prior to the child's first attendance or within one month after attendance. Evidence: the record for child #5 (date of first attendance: 10/2/23) did not have documentation of a physical examination.

Plan of Correction: The family had been notified that documentation of updated physical examination was needed. Physical was submitted by the family on 1/23/2024.

Standard #: 8VAC20-780-70
Description: (Repeat violation) Based on review of six staff records, not all records contained all required information. Evidence:
1) The record for staff #2 (date of hire: 12/20/21) and staff #6 (date of hire: 12/7/23) did not have documentation that two or more references were checked before employment.
2) The record for staff #1 (date of hire: 10/18/23), staff #2 (date of hire: 12/20/21), staff #4 (date of hire: 5/22/23) and staff #5: (date of hire: 11/7/22) did not have complete documentation to demonstrate that the individual possesses the education, certification, and experience required by the position, and orientation and training as required.

Plan of Correction: References for the staff members are on file. All documentation was completed on 1/18/24.

Standard #: 8VAC20-780-340-D
Description: (Repeat violation) Based on observation and review of documentation, the facility failed to ensure that in each grouping of children at least one staff member meets the qualifications of program leader. Evidence: staff #5 was observed working alone in the baby steps A classroom and does not have documentation of group leader qualifications (age/education/experience) in their record.

Standard #: 8VAC20-780-510-E
Description: Based on review of documentation and staff interview, it was determined that the facility failed to follow their own medication procedures as required. Long term prescription drug use may be allowed with written authorization from the child?s physician and parent. Evidence: the record for child #1, child #2 and child #4 did not contain written authorization from the child's parent or physician for long term emergency medication that was observed on site.

Plan of Correction: Short term medication authorization forms were received on the same day of inspection.
