Comments:
An unannounced renewal Inspection took place on January 22, 2024 between the hours of approximately 10:40 a.m. and 11:40 a.m. There were 3 classrooms observed with a total of 13 children with 5 staff within the supervision guidelines. The children were observed playing with manipulatives, engaging in free play and other organized activities. Positive interaction between staff and children were observed. A complete inspection of the physical plant, children and staff records, fire drill log, and attendance was conducted during this inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations:
Standard #: 8VAC20-780-130-A
Description: Based on record review, the center failed to obtain documentation that 1 out of 3 children received required immunizations before the child could attend the center.

Evidence: Child #1?s (start date: 05/05/2022) record did not contain documentation of immunizations.

Plan of Correction: Child was enrolled by previous director. Called the parent and current immunizations were brought upon pick up today.

Standard #: 8VAC20-780-140-A
Description: Based on record review, the center failed to ensure 1 out of 3 children had a physical examination before the child?s attendance or within 30 days after the first day of attendance.

Evidence: Child #1?s (start date: 05/05/2022) record did not contain documentation of a physical examination.

Plan of Correction: Child was enrolled by previous director. Called parent and asked for current physical. Will call doctor and get to TLC ASAP.

Standard #: 8VAC20-780-60-A
Description: Based on record review, the center failed to obtain all information for children?s records.

Evidence: Child #1?s (start date:05/05/2022) record did not contain documentation of viewing proof of the child?s identity and age.

Plan of Correction: Child was enrolled by previous Director. Called parent to bring proof of birth upon pick up today.

Standard #: 8VAC20-780-500-A
Description: Based on observation, the center failed to ensure that children's and staff hands were washed with soap and running water after a diaper change.

Evidence:
1. Staff #2 was observed changing Child #2?s diaper. Once the diaper change was complete, Staff #2 returned Child #2 to the play area.
2. Child #2?s hands were not washed before being returned to the play area.
3. Staff #2 was then observed placing Child #3 on the changing table for a diaper change.
4. Staff #2 did not was their hands between changing Child #2 and Child #3.

Plan of Correction: Had a meeting with the teachers to review the hand washing procedures after diaper changes. I also observed them to make sure they followed the correct handwashing procedures. Made the diaper changing poster detailing diaper changes more visible.

Standard #: 8VAC20-780-500-B
Description: Based on observation, the center failed to ensure that the diapering surface was cleaned with soap and water and sanitized after each use.

Evidence:
1. Staff #2 was observed changing Child #2?s diaper. Once the diaper change was complete, Staff #2 returned Child #2 to the play area.
2. Staff #2 was then observed placing Child #3 on the changing table for a diaper change.
3. Staff #2 did not clean and sanitize the changing table in between changing Child #2 and Child #3.

Plan of Correction: Had a meeting with the staff to review diaper changing procedures to ensure that the diapering surface is cleaned with soap and water and sanitized after each diaper change. Also observed them to make sure they were doing it correctly.
