Comments:
A renewal inspection was initiated on 1/23/2024 and concluded on 1/23/2024 from 11:45 AM to 1:50 PM. There were 64 children present, ranging in ages from infants to five, with 15 staff and three administrators. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies. A total of six children?s records were reviewed, two staff?s records were reviewed, and seven staff records were reviewed for updates.
Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 8VAC20-780-150-B
Description: Based on record review and interview, the center failed to obtain a physical and immunization for one out of six children records that was signed by a physician, his designee, or an official of a local health department.

Evidence:

1. The record for child 2 did not contain a physician or physician designee signature on the immunization and physical examination records.
2. Staff 3 reviewed the record and verified the documents did not contain the required signatures.

Plan of Correction: The parent will be notified by the end of the day to provide an immunization and physical record with a physician signature. In the future the center will review immunizations and physicals upon receipt to ensure all required information and signatures are obtained.

Standard #: 8VAC20-780-280-B
Description: Based on observation, the center failed to ensure hazardous substances were kept in a locked place using a safe locking method that prevents access by children.

Evidence:

In the infant room on a bottom shelf of an unlocked cabinet was a spray bottle of Lysol disinfectant, two spray bottles of Febreze Air Freshener and Clorox wipes (on top shelf of the same cabinet).

Plan of Correction: The hazardous substances were moved to a locked location during the inspection. All staff will be reminded of this standard.
