Comments:
An unannounced monitoring inspection was initiated on 11/8/2023 to investigate a self-reported incident regarding a forbidden action. The investigation was conducted jointly with a local agency. Documentation was reviewed, observation of the Twos classroom was conducted, and interviews were completed. The investigation concluded on 12/13/2023. A violation was found regarding rough handling, which is a forbidden action and is identified in the Violation Notice.

Standard #: 8VAC20-780-410-1
Description: Based on interviews, a staff member engaged in a forbidden action by roughly handling a child in care.

Evidence: In the Twos classroom, on 11/3/2023 at approximately 2:30pm, Child #1 (Age 2) was sleeping under a blanket on a cot when Staff #1 yanked Child #1 by the arm and roughly pulled Child #1 to a standing position. Staff #1 pulled Child #1 quickly across the room to the changing table. At the changing table, Staff #1 grabbed Child #1 by the upper left arm and roughly lifted the child onto the changing table.

Plan of Correction: We reported the incident to licensing and terminated the employee.
