Comments:
A renewal inspection was conducted Today, January 23, 2024. The licensing inspector was on site from approximately 9:55 am to 1:20 pm. The center director was on site and assisted with the inspection. There were 27 children present with five staff supervising. Children in care were observed reviewing the letter N, counting numbers, building blocks and dancing to music. Staff was observed singing and playing with an instrument for the infants. The inspector reviewed compliance in the areas of administration, physical plant, staffing, supervision, programming, injury reporting, medication, special care
and emergencies and nutrition. Medication is administered and was reviewed during today?s inspection. A total of five child records and three six staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22.1-289.036-A
Description: Based on a review of agent records, the center failed to ensure that one agent undergo a
background check in accordance with the Code of Virginia prior to issuance of a license
and every five years thereafter.

Evidence:

1. The record of Agent #4 contained documentation of a national fingerprint background check dated 04/03/2018, exceeding five years.
2. The director stated the applicant had not corrected missing information from the request to renew the national fingerprint check and therefor the national fingerprint had not been completed.

Plan of Correction: Per the director: The board officer needed to correct information on the request. I will have him complete the request form and submit it. We will follow up if it is not received in a timely manner.

Standard #: 8VAC20-770-60-B
Description: Based on review of six staff records, the center did not obtain a completed sworn statement prior to the 1st day of employment.

Evidence:

The record of staff #1, employed 08/10/2023, contained a sworn statement dated 08/11/2023 exceeding the 1st day of employment.

Plan of Correction: Per the director: I will have staff complete the sworn statement at the date of hire or before going forward.

Standard #: 8VAC20-770-60-C-2
Description: Based on review of six staff records and interview, the center did not obtain the results
of the central registry background report within 30 days of employment for one staff member.

Evidence:

1. The record of staff #1, employed 08/10/2023, contained the results of a central registry
background report dated 10/03/2023 exceeding 30 days from employment.
2. The director stated she did not have any additional documentation as to why the
background check was late.

Plan of Correction: Per the director: I will mail off the request for the central registry at the time of hire.

Standard #: 8VAC20-780-570-A
Description: Based on observation and interview, the center did not ensure when a child is placed in an
infant seat or highchair, the protective belt was fastened securely.
Evidence:

1. A child was observed being feed in an infant seat. The child did not have the protective seat belt fastened.
2. Staff acknowledged the protective seat belt was not fastened.

Plan of Correction: Staff buckled the seat belt while the inspector was on site. Per the director: I will review with staff our procedures to ensure children are fastened when in a seat.

Standard #: 8VAC20-780-570-E
Description: Based on observation and interview, the center did not ensure prepared infant formula was
refrigerated, dated, and labeled with the child?s name.

Evidence:

1. Two prepared bottles were observed setting on the table in the infant room. The two
bottles were not labeled with a name or date. Two additional prepared bottles were observed in the refrigerator without a date.
2. Staff acknowledged the bottles did not have a name and or date posted on them.

Plan of Correction: Per the director: I will review with staff to make sure bottles are checked for labels when families arrive. We will label any bottles that are not.
