Comments:
An unannounced monitoring inspection was conducted on 1/24/24 from 11:30am to 1:45pm as the result of a self-reported incident regarding forbidden actions. Interviews were conducted, the classroom was observed, and documentation was reviewed. One violation was issued as the result of the reported incident. Areas of non-compliance are identified in the Violation Notice.

Violations:
Standard #: 8VAC20-780-410-1
Description: Based on staff interviews and review of documentation, the center failed to ensure that the forbidden action of rough handling of a child did not occur.
Evidence:
1)Upon interview with staff, it was reported that Child #1 (age 4) refused to join the rest of the class for an activity on the rug. When the child refused, Staff #1 grabbed the child?s hand to guide them to the rug. In the process, the palm of the child?s hand became red and broken skin was noticed.
2)Upon pick up by the parent of Child #1, the child was crying, so the child?s parent asked their child what was wrong. Child #1 verbalized to the parent that Staff #1 had dug their fingernails into their hand.
3)Child #2 (age 5), who was present during the interaction, stated that Staff #1 had become upset with Child #1 and dug their fingernails into their hand when they weren?t listening. Child #2 stated that Child #1 cried when Staff #1 grabbed their hand.

Plan of Correction: From 1.22.24-2.1.24 the teacher was placed on administrative leave pending the investigation. In conclusion on the investigation the teacher received a written and documented final warning. The teacher is required to attend mandatory training on classroom supervision, student safety, and relevant courses determined by management.
