An unannounced self-report investigation was initiated on 1/23/2024 due to a self-report complaint made to the Department of Health and Safety. The investigation was completed on 01/24/2024. The inspector was on site on 1/23/2024 from 2:45pm until 4:15pm.
Two children's records and two staff records were reviewed. Interviews with staff were conducted as well.
Information gathered during the inspection determined the complaint to be NOT VALID. Non-related violations were issued to the program. Violations are documented on the violation notice issued to the program.

Standard #: 8VAC20-780-60-A Description: REPEAT VIOLATION
Based record review, the center failed to ensure that the separate record for each enrolled child shall contain all of the elements as required by the standards.
Evidence:
The record for Child #2 did not contain the following information:
Previous school attended, Food allergy and chronic physical problems.Plan of Correction: The PD will ensure that all information is in the child's file before enrolling. Parents have already completed and returned form.
