Technical Assistance:
Technical Assistance was provided to the Director regarding the timing of shelter-in-place drills.

Comments:
An unannounced renewal inspection was conducted on 1/25/24 from 9:00am to 10:55am with the Director. There were 21 children in care, ranging in age from three- to five-years-old, supervised by four staff. The children were observed rotating through structured centers. Five child records, three staff records, and two Board Officer records were reviewed. The Center has multiple staff with current certification in CPR and First Aid, as well as staff trained in Daily Health Observation. A sample of five medications and authorization forms were reviewed, and the center has two staff current in Medication Administration Training (MAT). Required postings were observed. The attendance and emergency drill log were reviewed. The first aid kit, flashlight, and battery-operated radio were observed. The most recent Fire Inspection on file was dated 5/17/23 and the most recent Health Inspection on file was dated 1/22/24. Please complete the "Plan of Correction" and "Date to be Corrected" areas on the Violation Notice for each violation cited and return to me by close of business on 2/2/24. Plans of correction should include steps to correct the noncompliance with the standard, and measures to prevent the noncompliance from occurring again.

Violations:
Standard #: 8VAC20-770-60-B
Description: Based on review of three staff records, the program did not obtain a sworn statement from one staff prior to employment. Evidence: There was no sworn statement on record for Staff #2, with an employment date of 8/29/23.

Plan of Correction: Director will create a checklist for new hires complete with due dates.

Standard #: 8VAC20-780-160-A
Description: REPEAT VIOLATION Based on review of three staff records, the program did not obtain the results of a tuberculosis (TB) screening within the 30 days prior to employment for one staff. Evidence: The TB test on record for Staff #2, with an employment date of 8/29/23, was dated 8/23/22.

Plan of Correction: Director will review all staff TB dates and include this on the new hire checklist.

Standard #: 8VAC20-780-80-A
Description: REPEAT VIOLATION Based on observation, the center did not maintain a written record of daily attendance that documented the arrival and departure of each child in care as it occurred for each group of children. Evidence: At approximately 9:15am on 1/25/24, staff in the three-year-old class reported that they had not completed attendance for the day, or several days prior.

Plan of Correction: Remind staff of requirement. Director will put on her monthly checklist to review attendance for all classes.

Standard #: 8VAC20-780-240-A
Description: REPEAT VIOLATION Based on review of three staff records, the program did not ensure that the Virginia Department of Education-sponsored orientation course (Preservice) was completed within 90 calendar days of employment for one staff. Evidence: The record for Staff #2, with an employment date of 8/29/23, did not contain documentation of completing Preservice training.

Plan of Correction: Director will place the dates for new hires to complete their preservice training on the new hire checklist.
