An unannounced monitoring inspection was conducted today in response to two self reported incidents at the facility being received in our office.
Based on multiple staff interviews, inspection of the playground, and review of injury reports, violations were issued,

Standard #: 8VAC20-780-80-B-2 Description: Based on a review of documentation and provider statement, the center failed to inform the department's representatives as soon as practicable, but not exceeding 2 business days of an injury that occurred while the child was under the supervision of the center, that required outside medical attention.
Evidence:
Child #1 sustained an injury on 12/28/2023 that required outside medical attention.
The Department of Education was not notified of the injury until 1/4/ 2024.Plan of Correction: The center will report any injury to the department's representative within 2
business. If the child requires outside medical attention.

Based on a review of injury reports, all required information is not documented.
Evidence:
2 of 2 reports reviewed did not contain documentation of either a parent and staff signature or two staff signatures. (Child #1)Plan of Correction: Both injury reports were signed on 1/29/24.
