An unannounced probationary inspection was conducted on-site on January 29, 2024. The director was available during the inspection. The inspector reviewed compliance in the areas of administration, staffing and supervision, and special care provisions and emergencies. Records were not reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Standard #: 22.1-289.011-F Description: Based on observation of posted information, the center failed to ensure that documents required by the Superintendent shall be posted in a conspicuous place on the licensed premises.
Evidence: The Notice of Intent to Deny Renewal Application for License provided by the Virginia Department of Education, signed by the Director of the Office of Child Care Health & Safety on December 21, 2023, requires posting of the Violation Notices attached to the Notice of Intent. These Violation Notices, dated March 20, 2023, March 30, 2023, April 14, 2023, and August 17, 2023, were not posted as required.Plan of Correction: The violation notices were posted to the NOI while the licensor was in the center.
Standard #: 8VAC20-780-500-A Description: Based on observation and interview, the center failed to ensure that staff's hands shall be washed with soap and running water before and after a diaper change and children's hands shall be washed with soap and running water after a diaper change.
Evidence: In the infant 3 room, staff #1 was observed changing a diaper. After the diaper change, staff #1 did not wash the staff's hands or the child's hands. Staff #1 placed the child on the floor to play with a toy and then picked up another child and held the hand of another. Staff #1 confirmed that staff and children's hands were not washed after the diaper change.Plan of Correction: Teacher was immediately retrained on proper handwashing by walking through the steps. Members of management team will observe handwashing procedures of teachers and will also ensure proper handwashing signs are posted near diaper changing stations.
