Comments:
A complaint was received by the licensing office regarding cleanliness and sanitation. An investigation was initiated on January 25, 2024. An on-site investigation was conducted on January 30, 2024. Observations were made and interviews were conducted. Based on the preponderance of evidence, the complaint is deemed valid. Violations cited can be found on the violation notice. Description: REPEAT VIOLATION

Based on observation, the center failed to ensure areas of the center, inside, were not maintained in a clean, safe and operable condition.

Evidence:
1. Witness A stated that daily around pickup time, the classroom restroom smells foul. They stated that urine is always on the floor and the toilet is rarely flushed.
2. On January 30, 2024, one out of two toilets in the restroom used by the Ladybugs classroom was observed to contain urine and toilet paper.
3. On January 30, 2024, toilet paper and paper towels were observed on the floor of the Ladybugs? restroom.
4. On January 30, 2024, a sippy cup was observed sitting on the toilet paper dispenser in a bathroom stall in the restroom used by the Ladybugs classroom.
5. On January 30, 2024, one out of two toilets in the restroom used by the Bees and Frogs classrooms was inoperable and contained feces and urine. There was an out of order sign posted on the door.
6. Staff #1 confirmed that the toilet had been inoperable since January 29, 2024 and a plumbing company was scheduled to come out on January 31, 2024.
7. On January 30, 2024, toilet paper was observed on the floor of a bathroom stall in the restroom used by the Bees and Frogs classrooms.

Plan of Correction: Ladybugs classroom has new transitions and children are being trained on bathroom etiquette. Teachers were advised to frequently check restrooms to ensure they are clean and children are flushing (following all steps of bathroom use). Teachers also check frequently to sweep any paper debris on the floor.

Teacher is working with one child who is attached to her sippy cup and would not let go.

Toilet that was blocked by a child going BM was immediately closed off and sign placed. No children were using that bathroom and there was another bathroom for use. Nightly cleaning company could not get to unblock the toilet that night, and hence a plumbing company was scheduled right away.

Description: Based on observation, the center failed to ensure when any surface has been contaminated with body fluids, it is cleaned and sanitized.

Evidence:
1. Witness A stated that daily around pickup time, the classroom restroom smells foul. They stated that urine is always on the floor and the toilet is rarely flushed.
2. On January 30, 2024, a puddle of liquid was observed on the floor of a bathroom stall in the restroom used by the Bees and Frogs classroom.
3. There was a urine odor in the restroom.
4. After dipping toilet paper into the puddle, it was determined that the liquid was urine.

Plan of Correction: The school is actively working with a child about the proper use of the bathroom and not peeing by the drain. Teachers are immediately advised that they should be checking the bathroom frequently to ensure it is clean, and if there are any puddles, they should call the office to get it cleaned and sanitized immediately.
