Comments:
A monitoring inspection was conducted on January 30, 2024 with the center director and staff . There were 55 school age children present, with 6 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 4 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-160-A
Description: Based on review of four staff records, the facility failed to obtain a tuberculosis test or screening from each staff at the time of employment and prior to direct contact with children. Evidence: Staff 3 (date of employment 6/05/2023) did not have documentation of a tuberculosis test or screening until 01/24/2024.

Plan of Correction: Staff will get a TB test or screening prior to coming in contact with children.

Standard #: 8VAC20-780-240-A
Description: Based on review of four staff records, the center did not ensure staff completed the Virginia Department of Education-sponsored orientation course within 90 days of employment.
Evidence: Staff 3 (date of employment 6/05/2023) did not have documentation of completing the Virginia Department of Education-sponsored orientation course.

Plan of Correction: Staff will complete the Va Pre-service training within 90 days of employment.

Standard #: 8VAC20-780-510-I
Description: Based on review of four medications, the facility failed to have written authorization from the parent and the child?s physician to administer medication. Evidence: There was no written parent/guardian or physician authorization to administer medication to Child A.

Plan of Correction: A long term medication authorization will be obtained or the medication will be returned.

Standard #: 8VAC20-780-510-P
Description: Based on review of 4 medications, the facility failed to notify parents that that the medication must be picked up within 14 days of the authorization expiring or that a new authorization should be completed. Evidence: The medication authorization for one medication expired 5/2/2023. The authorization had not been renewed and the medication was not returned or destroyed as required. The medication was on site.

Plan of Correction: The medication will be returned to the parent or destroyed.

Standard #: 8VAC20-780-560-J
Description: Based on observation, the facility failed to sanitize tables after each use for feeding. Evidence: the tables were not sanitized after afternoon snack. There were visible crumbs of granola bars on the table.

Plan of Correction: The tables were cleaned and sanitized during today's inspection.
