Comments:

A monitoring inspection was conducted on January 26, 2024. There were 6 children present, ranging in ages from 14 months to 22 months with 1 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 6 child records and 1 new staff record were reviewed. The children were having rest time. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Description:	Based on review of one new staff record, the facility failed to have documentation of a completed search of the central registry (CPS check) on file for each staff within the first 30 days of employment. Evidence: Staff 1 (date of employment 8/03/2023) did not have a completed search of the central registry on file. There was no documentation of having submitted the search of the central registry within 7 days of employment as required

Description:	Based on observation and interview, the facility failed to ensure during the stated hours of operation, there always shall be on the premises when one or more children are present one staff member who meets the qualifications of a program leader or program director and an immediately available staff member, volunteer or other employee who is at least 16 years of age, with direct means for communication between the two of them. Evidence: when the Licensing Inspector arrived at the center at approximately 2:00 p.m. on 1/26/2024, Staff 1 was the only person on site supervising 6 napping infants. A second staff did not arrive at the center until approximately 10 minutes later.

Description:	Based on observation and record review, the facility failed to maintain the ratio applicable to the youngest child in the group when children are regularly in ongoing mixed age groups. The infants and toddlers are in a combined group daily . Evidence: the youngest child is the group is 14 months. The ratio for infants is 1 staff for every four infants aged birth up to 16 months. Evidence: On 1/26/2024, there were 6 infant and toddlers with one staff person for approximately ten minutes.

Description:	(Repeat violation) Based on observation, the facility failed to use a leakproof or plastic-lined storage system that is either foot-operated or used in such a way that neither the staff member?s hand nor the soiled diaper touches an exterior surface of the storage system during disposal.
