An unannounced renewal inspection was conducted on January 30, 2024 from 10:30 a.m. through 12:00 p.m. Upon arrival, there were 22 children, ages 2 through 5, in the care of 4 staff.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 children and 5 staff records were reviewed.
Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Standard #: 22.1-289.036-A Description: Based on record review the facility failed to obtain a repeat fingerprint check for one board officer.
Evidence:
1. Five board officer records were reviewed.
2. Officer #1 did not have documentation of obtaining a repeat fingerprint checks within 5 years of the previous check.
3. Fingerprint check was documented on 05/03/2018.

Standard #: 22.1-289.036-B-2 Description: Based on record review the facility failed to obtain a criminal history check for two board officers.
Evidence:
1. Five board officer records were reviewed.
2. Board Officer #4 and #5 did not have documentation of obtaining a criminal history check within 30 days of becoming an officer.
3. Board Officer #4's date of was documented on the sworn statement as 11/11/2023. Board Officer #5's date was documented on the sworn statement 11/11/2023.
