Comments:
An unannounced inspection was initiated on July 6, 2023 with a representative of a local agency in response to a self-reported incident regarding a forbidden action. Interviews with staff were completed and documentation was reviewed.

The preponderance of the evidence gathered during the investigation supports the allegation; therefore, the self report is determined to be valid.

Standard #: 8VAC20-780-410-1
Description: On July 6, 2023, Staff 1 stated she observed Child A crying on her cot. Staff 1 asked Child A what was wrong, and she stated Staff 2 "hit her with a bucket". Staff 1 asked Child A where did Staff 2 hit her and the child said "on my back". A photo taken the day of the injury shows raised red marks on the child's lower back.
2. Staff 3 stated she was in the room next door when she heard Staff 2 telling Child A to "shut up, shut up!" and "Get on your cot!" She heard 4 loud bangs, saw Staff 2 standing over the child who was on her cot, and saying "If you think that hurt..."
3. Staff 4 stated she was in the room next door and heard Staff 2 yelling and heard loud bangs.
3. Staff 2 stated she was upset the day of the incident and did raise her voice at the students during nap time. Staff 2 stated Child A pulled something off of a nearby cubby and a bucket fell onto her.
