An unannounced monitoring inspection was conducted on January 30, 2024 from approximately 3:10 pm to 4:40 pm. Two staff were present supervising eight school age children. Children were observed participating in group led games and table activities. Four staff and five child records were reviewed. Violations were found and are noted on the violation notice.

Based on review and interview, facility did not ensure there is a completed central registry search dated within 30 days of beginning employment.

Standard #: 8VAC20-780-80-A Description: Based on review, facility did not ensure for each group of children there is a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.
