A self-reported monitoring inspection was initiated on 1/25/24 and concluded on 1/25/24. The inspector reviewed compliance in the area of programming.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-780-400-B Description: Based on a self-report, review of video footage, and review of documentation the facility failed to ensure that behavioral guidance is constructive in nature, age and stage appropriate, and redirects the children to appropriate behaviors.
Evidence:
Video footage for 1/24/24 showed staff #1 engaged in inappropriate behavior guidance with child #1 who is preschool aged.
At approximately 8:36am on 1/24/24 child #1 is seen taking a toy from another child. Staff #1 sees this and approaches child #1 and takes the toy and gives it back to the other child. Child #1 is then seen throwing a toy onto the table top because of this. Staff #1 then grabs child #1 by the upper arms and shakes the child aggressively several times while talking to him close to his face. The video has no sound, but staff statements stated that "staff #1 was heard telling the child we don't do that here, do you want me to smash your tablet?"
Staff #1then lets go of child #1 and the child goes to the ground crying.Plan of Correction: Review of the incident immediately began on 1/24/24 when the incident was reported. After review of all items related to the reported incident, proper internal procedures were implemented. All required agencies were notified of this incident as required.
A corrective action plan/quality improvement training was issued to all staff for completion to ensure future compliance with standards and the care of the children.
Standard #: 8VAC20-780-410-1 Description: Based on a self report, review of video footage, and staff statements the facility failed to ensure that physical punishment, rough handling or shaking a child does not occur.
Evidence:
Video footage for 1/24/24 shows staff #1 roughly handling and shaking child #1(preschool aged) approximately 3 times for behavior guidance.
At approximately 8:36 am staff #1 is seen grabbing child #1 by the upper arms and shaking him approximately 3 times while talking to him closely in his face.
Child #1 had taken a toy from another child, staff #1 saw this and walked over to child #1 and took the toy back and gave it to the other child. Child #1 then throws another toy because of this. Staff #1 then uses forbidden action, rough handling and shaking, as behavior guidance.Plan of Correction: Review of the incident immediately began on 1/24/24 when the incident was reported.
