Comments:
An unannounced monitoring inspection was conducted on 2/1/2024 at 1:45pm. There were 47 (19 mos to 5 years) directly supervised by 9 staff. The physical plant, 3 staff records, 5 children?s records, 3 children's medications and medication authorization records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed napping, waking up from nap, and dismissal. Diapering and hand-washing procedures were also observed. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in this report. Standard #: 8VAC20-780-40-J
Description: Based on review, the facility did not have documentation of an injury prevention procedures that are updated annually based on documentation of injuries and a review of the activities and services.
Evidence: reviewed the facility records and found that there was not an annually updated injury prevention plan.

Plan of Correction: 2023 Annual Injury Prevention Plan is attached (emailed to the inspector)

Standard #: 8VAC20-780-270-A
Description: Based on observation, the facility did not ensure that areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.
Evidence:
1) observed 2 small nails protruding from the stone/window sill in the primary room.
2) observed a crack in the stone wall large enough for children to fit their fingers in the primary room.
3) observed 2 large screws protruding from the fence on the younger playground.
4) observed peeling paint along the lattice/fencing on the playground.

Plan of Correction: Nails in the pre-primary room were moved and the two large screws on the fence were removed. Peeling paint along the lattice/fencing on playground and the cracks in the stone wall are being repaired the weekend of February 10 & 11, 2024 unless weather prevents painting. If weather prevents painting handyman will paint the next dry weekend.

Description: Based on observation, the facility did not ensure that hazardous substances are kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1) observed hand sanitizer accessible to children in all 4 classrooms.
2) observed hand lotion labeled "keep out of reach of children" and Lysol spray in an unlocked cabinet, accessible to children in the 3s classroom.
3) observed "Stress EZ pure essential oils" marked "keep out of reach of children" on the window sill.
4) observed miracle gro, labeled "keep out of reach of children" in an unlocked cabinet, accessible to children.

Plan of Correction: Hand sanitizer bottles (in all rooms) were (put) in child lock cabinets or locked closets February 1, 2024. The hand lotion and Lysol in room 3 were put in the classroom's locked closet on February 1, 2024. Essential oil in room 4 was secured in child locked cabinet on February 1, 2024. Miracle Grow was put in the locked basement on February 1, 2024.

Description: Based on observation, the facility did not ensure that diaper ointment or cream was labeled with the child's name.
Evidence: observed a small container of diaper ointment/cream that was not labeled with the child's name.

Plan of Correction: Diaper ointment was labeled with child's name on February 1, 2024.

Standard #: 8VAC20-780-540-B
Description: Based on observation, the facility did not ensure that the first aid kit was easily accessible to staff but not to children.
Evidence: observed a first aid kit in a backpack on a chair in the kindergarten classroom that was accessible children.

Plan of Correction: First aid kits were checked - scissors and hand sanitizers were removed February 1, 2024.

Standard #: 8VAC20-780-550-P
Description: Based on review, the facility did not ensure that written records of children's injury records were maintained with the required information.
Evidence: reviewed 10 injury reports and found that 7 of the reports did not have documentation of the time parents were notified.

Plan of Correction: All accident/incident reports will have the exact time and not "at pickup" as time parents were notified as of February 1, 2024.
