Technical Assistance:
Discussed the importance of properly fitted sheets for infant cribs that prohibit the crib mattress from pulling away from the sides of the crib.

The state model form for medication authorization will be provided as a resource.

Suggested a consistent annual date to review children?s files with parents to ensure an annual review of emergency contacts is made.

Comments:
An unannounced Monitoring Inspection was conducted today. A total of 85 children with 16 staff were onsite in addition to Administrative staff. A total of 9 classrooms were occupied today. Programming in all 9 classrooms was observed. Four infants were present in the Infant classroom and sleeping in their assigned cribs. Staff was observed holding and rocking an infant who had awakened. The classrooms were decorated with projects and artwork the children had completed such as painted snowmen, class flags and handprint canvas. Table top activities such as wooden puzzles and a tactile activity of paint in ziplock bags were engaging the children in the Sea Lion and the Black Footed Feret class. Curriculum on bird migration was taught in the Red Panda class that included construction paper bird nests made by the children. Handwashing procedures were observed for both staff and children as the children transitioned from morning activities to lunch. School lunch today consisted of a choice of hamburgers or chicken with corn and fresh bananas with milk to drink. Some medication needs identified today which included medication onsite as well as topical ointments in some classrooms. Positive interaction was observed with a staff and one child who was adjusting to being newly enrolled. After lunch, the children transitioned to nap time as staff were observed laying out cots and linens. A total of 5 children?s records and 6 staff records were reviewed which included new staff hired since the last licensing inspection in September. The previous violations found during the last inspection were reviewed and the corrective actions were verified today. This inspection occurred between 10:15am and 1:45pm. Description: Based on child record review, documentation of additional immunizations once every six months for children under the age of two years was not obtained.

Evidence:
1.Child # 3 is under the age of 2 years.
2.The most recent immunizations were dated 02/14/2023.

Plan of Correction: We will request an updated immunization record from the parent.

Description: Based on child record review, 2 out of 5 records reviewed were incomplete.

Evidence:
1.Child #4?s file was missing 1 emergency contact name, address and phone number.
2.Child #2?s file was missing the address for one emergency contact.

Plan of Correction: This information will be requested from the parents and maintained in the children's files.

Standard #: 8VAC20-780-245-M
Description: Based on staff record review, documentation of training was not kept by the center in a manner that included the training delivery method;
the entity or individual providing the training and the number of training hours or credit hours received.
Evidence:
The documentation of training for Staff # 3, 4 and 5 did not consistently include the length of the training, the individual providing the training and the delivery method.

Plan of Correction: A training log will be developed that includes all required documentation to ensure a total of 16 hours of annual training or staff qualifications are maintained.

Standard #: 8VAC20-780-270-A
Description: Based on observation, areas of the center were not maintained in a clean and safe condition.

Evidence:

1.The corner of the half wall located by the exterior window in the Red Panda room had chipped paint exposing a sharp metal corner presenting a hazard to children in care.

2. The diaper changing tables located in the Saola and Rhino rooms were observed dirty with accumulated debris on the side edge between absorbent surface and side of the table.

Plan of Correction: A corner bumper was added to the sharp corner wall. Corrected on site.

Staff will be re-trained to clean the diaper changing tables.

Standard #: 8VAC20-780-440-B
Description: Based on observation and staff statements, cots were not identified for use by a specific child.
Evidence:
Cots found in the Soala classroom were not identified for use for several children. The lead teacher in the Saola classroom stated they were unable to identify cots for specific children because the labels with child names had either peeled or fallen off.

Plan of Correction: Cots will be labeled by tomorrow.

Standard #: 8VAC20-780-500-B
Description: Based on observation, the diapering surface was used in a manner other than diapering or cleaning children.
Evidence:
The diapering surface in the Rhino classroom had teacher materials laying on the surface

Plan of Correction: Teacher materials were removed from changing table and corrected on site.

Standard #: 8VAC20-780-530-A
Description: Based on review of staff records, one classroom did not have a staff with current First Aid certification.
Evidence:
1.Staff #2 was the only staff present in the Infant classroom.
2.Staff #2 does not have current First Aid certification.

Plan of Correction: Another staff with current certification returned to the classroom.

Standard #: 8VAC20-780-560-F-4
Description: Based on observation, children three years of age or younger were offered foods that are considered to be potential choking hazards.

Evidence:
1) Children in the Saola classroom were observed eating whole bananas broken in half.
2) Children in the Rhinos classroom were served and observed eating uncooked baby carrots, whole bananas broken in half, full chicken strips, and full hamburgers.

Plan of Correction: Staff will be re-trained today on potential choking hazards. We will be getting choke tubes for our classrooms.

Description: Based on observation of classrooms, food brought from home was not properly labeled.
Evidence:
Food containers and infant bottles brought from home that was found in the refrigerator in the Infant classroom were not labeled with either a child?s name or date.

Plan of Correction: We will make sure this is corrected today and moving forward.

Standard #: 8VAC20-780-560-N
Description: Based on observation, food was not stored in a sanitary manner.

Evidence:
A yogurt with the label to ?keep refrigerated? was stored in a lunch box inside a cubby and not refrigerated in the Black Footed Ferret class.

Plan of Correction: This food was removed immediately. Staff will inspect food brought from home to ensure they are stored with ice packs if needed.
