Comments:

A monitoring inspection was conducted on October 16, 2023. The inspector was on site from approximately 3:00 pm until 4:45 pm. The site director was present during the inspection. There were 20 children present, ranging in ages from six years to ten years, with two staff supervising. Children were observed in a variety of activities including a game of four corners, books, blocks and coloring. Later children worked on a group project designing their own monster. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five children?s records were reviewed. Three staff records were reviewed remotely on October 19, 2023. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #:	8VAC20-770-60-C-2
Description:	Based on a review of three staff records and interviews, the center did not ensure a completed central registry was obtained within 30 days of employment for one staff member. Evidence: The record for Staff #2, employed on 08/21/2022, contained the results of a central registry finding dated 12/07/2022 exceeding 30 days from the date of employment. The record did not contain documentation of any further contact or follow up as to the status of the request and the staff has been continuously employed. A member of management stated there was no documentation of a follow up to the status of the request.
Plan of Correction:	Per the director: Going forward, We have systems in place to ensure a follow up is documented when checks are not returned within the 30 days from employment.
Standard #:	8VAC20-780-60-A
Description:	Repeat Based on review of five children?s records, the center did not obtain all required documentation for two children?s records. Evidence: 1. The record for child #1, enrolled 08/22/2023, did not contain the name, address, and phone number for one emergency contacts. The record also did not contain the work phone number and employer for each parent listed. 2. There was no record on site for child #3. Child #3 was in care during the inspection on 10/16/2023.
Plan of Correction:	Per the director: We are working on reviewing all children's records to obtain any missing documentation. We had the record of child #3, it however had not been uploaded to our program and therefore was not on site.
Standard #:	8VAC20-780-240-A
Description:	Based on review of three staff records, the center did not ensure one staff completed the Virginia Department of Education sponsored orientation course within 90 calendar days of employment. Evidence: The record of staff #2, employed 08/21/2022, did not contain documentation of completion the Virginia Department of Education sponsored orientation training, exceeding 90 from employment.
Plan of Correction:	Per the director: I will have the staff complete the training.
Standard #:	8VAC20-780-245-L
Description:	Repeat Based on review of three staff records, the center did not ensure there was always at least one staff member on duty who has obtained within the last three years instruction in performing the daily health observation of children. Evidence: The record of staff #1 and staff #2 did not contain documentation of completing health observation training. Staff #1 and staff #2 were the only staff on duty during the inspection on October 16, 2023.
Plan of Correction:	Per the director: We will have all staff complete daily health training to ensure there will be a staff on site with the training.
Standard #:	8VAC20-780-260-A
Description:	Repeat Based on review of documentation and interview, the center did not obtain an annual fire inspection report. Evidence: The most recent fire inspection report was dated 03/03/2022, exceeding the annual requirement. Staff #1 stated they did not have a current fire inspection report.
Plan of Correction:	Per the director: We will contact the school and fire Marshall to obtain a current fire inspection.
